Form DD 3150
Recent guidance released by the Deputy Secretary of Defense requires that all travelers to the U.S. Naval Submarine Base New London attest to their vaccination status via DD Form 3150. Please note all attendees and speakers must carry a completed DD Form 3150 that must be kept on their person and shown at the conference check-in.
Industry attendees who DO NOT have a DoD ID number, please leave this section blank.
Proof of Vaccination:
Additionally, NDIA is requiring all attendees will be required to be fully vaccinated and provide proof of vaccination.
An individual is considered fully vaccinated 14-days after the second dose of a two-dose vaccine series (Pfizer-BioNTech, Moderna, AstraZeneca-SK Bio) or 14-days after a single-dose vaccine (Johnson & Johnson/Janssen). All World Health Organization-approved vaccines are accepted.
